Goal-Setting and Therapy Plan

Based on your assessment, your OT collaborates with you to set practical, meaningful goals. Together, you develop a personalised therapy plan with clear steps and timeframes. This plan is yours. 

Step 4:

Ongoing Therapy and Progress Review

Your OT delivers regular therapy sessions at your home (or via telehealth) to work towards your goals. We track your progress, adjust the plan as needed, and celebrate your achievements along the way. 

Funding Options for Occupational Therapy
